The great Saint of Mantralayam Sri Guru Raghavendra swamy is known to have performed penance here for twelve years inside this temple which is a cave. Daily he worshipped Lord Sri Rama in that cave. After that, Sri Raghavendra swamy was doing medidation. While doing meditation he had carved the images of Lord Venkateswara and Sri Padmavathi devi on the rock inside the cave.
One day Lord Sri Anjaneya swamy blessed Sri Raghavendra swamy appearing before him with five faces ‘panchamukha’. The five faces of the Lord were Hanuman, Hayagriva, Narasimha, Garuda and Varaha. The image of the Lord Anjaneya in this unique form had been carved inside that cave. Hence This deity is known as 'Panchamukhi Hanuman'.
